Free transition goals worksheet for high school

Transition planning starts with what the student wants. This free set walks a high school student through what they like to do, then learning, working and living after high school, what they are good at, and who can help, with a job skills check at the end. It is a starting point for the transition goals in the IEP.

What I Like to Do: a checklist of kinds of work (working with my hands, helping people, being outdoors, using computers and more), each marked I like it, Not sure or Not for me.

What it is

A student-centered transition planning set: an interest checklist, a My Future page for learning, working and living after high school, and a job skills check.

How to use it

  1. Do the interest checklist first; there are no wrong answers.
  2. Fill My Future together, in the student's words.
  3. Use the answers to draft postsecondary goals with the team.

Three quick tips

  • Let the student say, write, draw or use their device to answer.
  • Talk with the family about the living section.
  • Come back to it every year; plans change.
