Accommodations Checklist: tables for presentation, response, setting and timing, such as directions read aloud, extended time, a word bank and small group, each with boxes for in the plan and used this week, and a use log with a column for each school day.

Common classroom accommodations in four groups (presentation, response, setting and timing), each with a box for in the plan and used this week, then an accommodations use log for a week (used, refused or not needed each day) to bring to the next team meeting, and a page for the adult on using it well. Initials only.

What it is

A checklist for general education and special education teachers who need to see a student's IEP or 504 accommodations at a glance and keep track of what was used.

How to use it

  1. Check only what the student's IEP or 504 plan lists.
  2. Keep it with your plans and check what you used each week.
  3. Log a week of use and bring it to the team meeting or to the case manager.

Three quick tips

  • Offer an accommodation to the whole class when you can, so the student isn't singled out.
  • Write refused when a student says no; that is useful for the team.
  • Use initials only on the page.

Questions

Does it decide what accommodations a student gets?
No. The IEP or 504 plan decides that. This page helps you follow the plan and track it.
What is the difference between an accommodation and a modification?
An accommodation changes how a student learns or shows what they know. A modification changes what they are expected to learn, and only the team decides that.
Is there a page for the student?
Yes. There is a free My Accommodations self-advocacy page for the student to use.

A starting point for your own professional judgment. Use initials only on any page about a student.
