Free articulation data sheet, trial by trial
Mark each production as it happens: + for correct on their own, P for correct with a prompt, − for incorrect or no response. Each row is one target (a sound in a position, or a word), with ten trials and a column for the percent correct.

Make your own in seconds
Ask for a data sheet for any skill, with more or fewer trials, or a progress tracker across sessions, and SCAFLI makes it.
Try asking for:
- “articulation data sheet”
- “IEP goal progress tracker”
SCAFLI is in early access. The free PDF above needs no sign-up.
What it is
A session data sheet for speech sound practice. It records each trial, separates prompted from independent productions, and shows how to work out percent correct.
How to use it
- Write the target in the first box of the row (for example, initial /s/ words).
- Mark each trial as + independent, P prompted or − incorrect.
- At the end, count the + marks and fill in the percent.
Three quick tips
- Write the goal at the top so anyone can pick up the sheet.
- Circle the prompt you used most, so the next session starts there.
- Use initials only on the sheet.
Questions
- Do prompted trials count as correct?
- This sheet counts only independent trials toward the percent, and records prompted ones separately.
- Can I track more than one sound?
- Yes. Each row is its own target, so one sheet can hold several sounds or positions.
- Is it a finished assessment?
- No. Like every SCAFLI data sheet, it is a draft for your own professional review.
A starting point for your own professional judgment. Use initials only on any page about a student.
