What it is

A landscape data sheet for one goal: trial by trial, session by session, with prompt level codes (independent, gestural, visual, verbal, model, physical) and a place to plan the next step.

How to use it

  1. Write the goal at the top, in the words from the IEP.
  2. Each session, write a code for each trial and count the independent ones.
  3. Every few sessions, check the next-step box to fade cues or change the setting.

Three quick tips

  • Keep one sheet per goal.
  • Use the carryover box to note where the skill showed up outside the session.
  • Initials only on the sheet.
