Free check-in check-out (CICO) daily point sheet

A one-page daily point sheet for check-in check-out. The student checks in with a mentor in the morning, carries the sheet, and each teacher rates three goals (be safe, be respectful, be ready to learn) for each part of the day. At check-out, the points are added up against the daily goal and the student writes what went well.

My Daily Point Sheet: student initials, date and daily goal; a table of three goals (be safe, be respectful, be ready to learn) and teacher initials across morning, reading, math, lunch and recess, specials and afternoon; points today and a check-out line.

What it is

A daily progress report for check-in check-out, a common Tier 2 behavior support in schools that use PBIS, from elementary through middle school.

How to use it

  1. Check in each morning: go over the goals and the daily goal together.
  2. Each teacher rates the goals at the end of their period: 2 yes, 1 some of the time, 0 not yet.
  3. Check out at the end of the day: add the points, praise what went well, and send a copy home if that is part of the plan.

Three quick tips

  • Keep the ratings quick and the tone positive.
  • Set a daily goal the student can reach at first, then raise it.
  • Look at a few weeks of sheets together before changing the plan.
